At your first appointment, we can address any concerns, answer any questions, go over scheduling and payment options and give you information on how to submit to insurance!
Patient calls our office to schedule an appointment
OR
A doctor send us a referral and we call the patient to schedule
Once the patient’s appointment is scheduled. They will be sent a link to fill out online paperwork.
This must be completed 24 hours prior to the first visit.
A confirmation text is sent a few days prior to the patient’s appointment.
The patient shows up ready for their functional vision exam!
What To Bring to Your First Appointment
- Completed Paperwork
- Copy of your insurance card (so we have it on file if needed)
- Any paperwork from other clinics you’d like for us to have on file
- Payment for the visit
